1. Date and place of execution:

17 March 1943 in Chmielnik, at the Jewish cemetery.

2. Type of execution (shooting, hanging, etc.):

Shooting.

3. Personal data of the murdered victims:

Poles, Jews, foreigners: Poles.

Number of murdered victims: 16 men.

The victims were brought in from: Kielce, by the Gestapo and the gendarmerie.

Surname, age, occupation, address: The surnames are unknown; all men were from 20 to 35 years old.

4. Is it known what the victims were accused of, whether the execution was carried out in retaliation, etc.?

According to German announcements, the execution was carried out in retaliation.

5. Who carried out the execution (gendarmes, Gestapo men, SS men, policemen, the Wehrmacht)?

Gendarmes, Gestapo men and SS men from Kielce.

6. Are the surnames of the perpetrators known (provide the surnames if known)?

Unknown.

7. Were the corpses burnt or destroyed otherwise?

They were buried at the execution site.

8. Where were the corpses buried (provide the exact location)?

At the execution site, i.e. the Jewish cemetery in Chmielnik.

9. Description of the place of burial (graves, dimensions, estimated number of victims buried in one grave):

There were 16 victims in one grave.

10. Were the corpses exhumed, were they officially reinterred (report)? If “yes” to the latter, then where?

The families took 14 corpses, and the remaining two are still buried at the execution site.

11. Are there any grounds for conducting their exhumation in the future?

It would be difficult to conduct their exhumation, because later on the Germans buried Jews there.

On 29 September 1945, the contents of the above questionnaire were officially confirmed by the mayor of the town of Chmielnik, Stefan Dąbski, with the Municipal Court in Chmielnik.
